**Job Title: Remote Healthcare Consultant**

**Location:** Work from Home, Miami-Dade County, FL

**Language Requirements:** Bilingual in Spanish and English

**About the Role:**

We are seeking multiple friendly, professional, and bilingual Remote Healthcare Consultants to join our team! In this role, you'll coordinate case management activities for Long Term Care/Comprehensive Program enrollees, facilitating appropriate healthcare outcomes for members through care coordination, support, and education.

**Responsibilities:**

- Utilize critical thinking and judgment to collaborate and inform the case management process.
- Conduct comprehensive evaluations of members using care management tools and data reviews.
- Coordinate and implement assigned care plan activities, monitoring progress along the way.
- Conduct multidisciplinary reviews to achieve optimal outcomes for members.
- Identify and escalate quality of care issues through established channels.
- Utilize negotiation and motivational interviewing skills to secure appropriate services and promote lifestyle/behavior changes for members.
- Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices.
- Help members actively participate with their providers in healthcare decision-making.
- Utilize case management and quality management processes in compliance with regulatory and accreditation guidelines.

**Requirements:**

- Bachelor's degree in Social Work or a related field (nursing degrees will not be considered for this role).
- Proven case management experience, with long-term care experience preferred.
- Competency in Microsoft Office, including Excel.
- Ability to multitask, prioritize, and effectively adapt to a fast-paced, changing environment.
- Excellent communication skills, both verbal and written.
- Residence in Miami-Dade County, FL.
- Ability to travel approximately 75% of the time within the region to visit members at home, in assisted living facilities, and nursing homes.

**Training:**

- Training will be conducted remotely via Microsoft Teams for approximately 4 weeks.

**Note:**

- Candidates must reside in Miami-Dade County, FL.
- Bilingual proficiency in Spanish and English is required for this role.
