About the Role:
As a key member of our integrated care team (which includes an RN and a Social Worker), you'll provide personalized, compassionate medical care to patients with CKD and/or ESRD/ESKD. You will play a crucial role in helping them improve their quality of life and slow the progression of kidney disease. This role offers flexible scheduling with a hybrid and in-home care model. This is a remote position within your state, with occasional travel for training and meetings.
What You'll Do:
Conduct comprehensive assessments, including annual wellness exams, both in-home and virtually.
Counsel and educate patients and families about available benefits and programs.
Maintain accurate and current patient records, documenting chief complaints, diagnoses, medical history, reviews of systems, examinations, medications, allergies, assessments, and plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation promptly.
Maintain high-quality care standards.
Deliver evidence-based, timely care to reduce avoidable hospitalizations and maximize quality of life.
Prescribe medications, order tests, and collaborate with the patient's physician.
Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Make assessments of patients' health status.
Develop and implement treatment plans.
Follow up and evaluate patient status.
Perform other duties as assigned.
What You'll Need:
Active and unrestricted RN and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs and information systems.
Excellent communication skills (verbal and written).
Understanding of federal and state regulations (HIPAA and OSHA).
2+ years of direct patient care experience.
Experience with managed care/IPA/health plans.
Experience conducting annual wellness visits or similar comprehensive visits (virtually or in-home).
Bilingual (English/Spanish) is highly preferred.
