If so, we're seeking a dedicated Mental Health Practitioner to join our Assertive Community Treatment (ACT) team.

What You'll Do:

Empower individuals: Provide comprehensive, integrated care to adults living with mental illness, substance use disorders, and co-occurring conditions.
Build strong relationships: Work closely with clients in their homes, at work, and in other community settings to develop trusting and supportive relationships.
Develop personalized treatment plans: Collaborate with clients and interdisciplinary teams to create individualized treatment plans that address their unique needs and goals.
Advocate for your clients: Assist clients in accessing essential resources, such as housing, healthcare, and employment.
Provide crisis intervention: Respond quickly to client needs and provide support during times of crisis.
Engage in ongoing professional development: Enhance your clinical skills and stay abreast of the latest evidence-based practices.

Qualifications:

Bachelor's or Master's degree in Behavioral Sciences or a related field.
2,000 hours of supervised experience working with individuals with mental illness.
Strong communication, interpersonal, and organizational skills.
Valid driver's license and reliable transportation.
