BROAD OVERVIEW:
The primary purpose of this position is to provide Patient Centered Care, be the patients advocate and to coordinate the RAI process assuring the timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan. The ideal candidates must be devoted and compassionate about their chosen career and is passionate about patient care. The MDS Coordinator is the patient advocate and bridges the communication between patient, family, and facility staff. This is a rare opportunity to join a solid long term care team!
MAJOR RESPONSIBILITIES:
- Complete accurate MDS & initiate care plans (as assigned)
- Create and distribute monthly care plan calendars in a timely fashion
- Monitor & audit clinical records, ensuring accuracy & timeliness
- Informing DON of persistent issues related to non-compliant documentation
- Protect the confidentiality of Resident & Facility information at all times
- Responsible for management of case mix process and overseeing IDT involvement in the MDS process
- Evaluate residents for Medicare Part A coverage prior to and upon admission
- Schedule screening and determination of coverage for all residents who may qualify for Medicare Part A
- Maintain Minimum Data Set (MDS) compliance per RAI guidelines
- Coordinate scheduling for all MDS’s and timely submission to government agencies
- Perform medical record review as requested by CMS/HMO’s using their guidelines
- Oversee/assist nursing staff responsible for maintaining compliance of MDS driven care plan updates and changes
- Train, update, and manage IDT in MDS expectations and care planning
- Responsible to review quality measures, report those findings in monthly QA and manage/overseeing quality systems
- Develop and update assigned policies and procedures; Ensures nursing department compliance with Medicare, Medicaid, and other insurance reimbursement regulations, including but not limited to documentation
- Ensure the completion of assessments in a timely manner
- Involvement and supervision of inputting of MDS assessments, their transmission, and validation
MINIMUM QUALIFICATIONS
- Must hold a Nursing Degree from an accredited college, university or trade school
- A current active RN or LPN license in NYS
- At least 2 years’ experience in Long Term Care as an MDS Coordinator
- RAC-CT preferred but not required
SKILLS AND QUAILITIES OF MDS COORDINATOR:
- Must have prior MDS 3.0 experience
- Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
- Excellent written and verbal communication skills and critical thinking abilities,
- Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facilities
- Ability to organize and prioritize.
- Significant understanding of 5 Star, VBP and QRP quality measures programs
- Should be friendly and a team worker
