The Department of Psychiatry & Behavioral Sciences at Montefiore Medical Center is recognized as one of the best in the nation. The Outreach Specialist will assist individuals who qualify for the High-Risk Care Management Program (HRCM). The HRCM program is intended to assist high acuity patients transition from inpatient to outpatient care. The HRCM program is a component of the Behavioral Health Centers of Excellence award.
The Clinical Manager manages and directs clinical and administrative activities for the Department of Psychiatry and Behavioral Sciences and The High-Risk Care Management Team.
Oversight of intensive care management team delivering services to eligible high risk individuals following discharge from MH/SUD inpatient/ER in the month following discharge, and the 3 months following discharge. Responsible for management of daily clinical operations and direct supervision of social workers and outreach specialist (peers).
Identify, onboard and train new high risk care management team.
Develop clinical protocols and clearly defined workflows for high-risk care management team across key clinical scenarios with key inclusion/exclusion criteria for intensive care management.
Responsibilities:
• Direct, manage and oversee daily clinical and administrative operations of practice
• Oversee intensive care management team delivering services to eligible high risk individuals following discharge from MH/SUD inpatient/ER in the month following discharge and 3 months following discharge
• Design and implementation of clinical initiatives for intensive care management
• Manage daily clinical operations and supervise social workers and outreach specialists. Direct clinical and administrative supervision of social workers and outreach specialists. Responsible for performance appraisals, monitoring of time and attendance, direct supervision, progressive discipline, and clinical oversight of staff
• Identify, onboard and train new high risk care management team
Qualifications:
• Master’s degree Social Work or equivalent Social Work required
• LMSW or equivalent; NYS licensed Social Work, Psychology or equivalent. At least 2 years clinical experience required
• Must possess strong clinical, analytical and communication skills
• Good working knowledge of program functions that include compliance with documentation and billing regulations
• Excellent leadership and time-management skills
• Knowledge of clinical modalities that may include psychodynamic psychotherapy, cognitive behavioral therapy, dialectal behavior therapy, and/or other evidence-based models required
• Knowledge for DSM-V diagnostic criteria
• Must have experience with competency in behavioral health needs of children age 4-17, and adults of all developmental ages
Must be able to work at other campuses based on departmental needs.
This is a 100% grant funded position.
